If you notice something on campus that requires maintenance or repair, please report using USC's online maintenance management system (login required) as well as completing an incident/hazard report form.

At times an incident or near miss will require further investigation. The purpose of such an investigation is:
- to ascertain the contributing factors of an incident, near miss or hazard and determine corrective actions
- as a legislative requirement, to record the details surrounding an incident and the follow up of actions implemented
- for insurance purposes, as part of an insurance claim, worker compensation claim or common law proceeding
The findings or outcomes and subsequent recommendations of an incident investigation, when implemented should prevent a reoccurrence of the incident, near miss or hazard. Refer to USC's Incident investigation guidelines, for details on this process.
