These forms are for domestic students only. If you are an international student seeking an application form, please refer to the international student application forms page.

Certain application forms will require supporting documentation as detailed on the form.
Tertiary Preparation Pathway
- Application for Tertiary Preparation Pathway (TPP) – Queensland Tertiary Admissions Centre (QTAC)
Application for admission to the Tertiary Preparation Pathway (TPP) is made on-line through the QTAC website.
Exception: Currently deferred TPP students must apply directly to Student Administration to take up a their deferred USC place in the Tertiary Preparation Pathway (TPP).
- Application for Tertiary Preparation Pathway (TPP) – direct entry for deferred applicant
This form is used by deferred applicants wishing to take up their deferred place in the Tertiary Preparation Pathway (TPP).
Note: All other applications for admission to the TPP are lodged on-line through the QTAC website
Undergraduate programs
- Application for undergraduate degree or associate degree – Queensland Tertiary Admissions Centre (QTAC)
Application for admission to undergraduate degree or associate degaree programs are made on-line through the Queensland Tertiary Admissions Centre (QTAC) website.
Exceptions: The below applicants must apply directly to Student Administration and refer to Application for undergraduate program – direct entry:
- eligible Tertiary Preparation Pathway students wishing to apply for a USC undergraduate program; or
- USC / TAFE articulation program students; or
- currently deferred applicants wishing to take up their originally deferred place.
Note: Deferred applicants wishing to apply for admission into a different USC program are required to apply online through QTAC.

- Application for undergraduate degree or associate degree – direct entry
This form is used by the following applicants:
eligible Tertiary Preparation Pathway (TPP) students wishing to apply for USC undergraduate program; or
- USC / TAFE articulation program students; or
- currently deferred applicants wishing to take up their originally deferred place; or
- completed an undergraduate degree within the last twelve months at the USC
Note: All other applications for admission to undergraduate degree or associate degree programs are made through the Queensland Tertiary Admissions Centre (QTAC) website.
- Application for USC undergraduate degree at South Bank
This form is for domestic applicants who are completing one of South Bank Institute of Technology (SBIT) Diploma listed below and wanting to apply for entry to a USC undergraduate program offered at South Bank.
Honours programs
- Application for Bachelor of Social Science (Psychology) (Honours)
- Application for all other honours programs
This form is used to apply for USC honours programs.
Postgraduate coursework programs
- Application for Graduate Diploma in Education
Applications for admission to the Graduate Diploma in Education are made through the Queensland Tertiary Admissions Centre (QTAC) website.
Exception: Students currently enrolled in or have completed study in a USC degree program in the last twelve months must apply directly to Student Administration by completing and submitting the Application for postgraduate coursework (PDF 232KB) *.
- Application for Master of Psychology (Clinical)
To apply for admission to the Master of Psychology (Clinical), you are required to submit relevant additional forms as listed below.
- Application for all other postgraduate coursework
This form is used to apply for all other USC postgraduate coursework programs.
Higher degrees by research

Headstart Program
- Application for Headstart Program
This form is used by Year 11 and 12 students to apply for USC courses while still completing high school.
Cross-institutional study
- Application for cross-institutional study
This form is for students currently studying in other institution and wishing to enrol in USC course(s).
Note: If you are a current USC student and wish to undertake study at another institution, complete a cross-institutional approval form, available from the student forms and document requests page.
Visiting students
- Application for visiting student
This form is used to apply directly to the University to complete one or more individual courses (subjects) for professional or personal development, rather than enrolling in an undergraduate degree program.
Credit transfer and recognition of prior learning
- Application for credit transfer
This form is to apply to transfer your previous completed study from another institution to your USC program. Completed form can be attached to the application form.
- Recognition of prior learning
Recognition of prior learning (RPL) is the process of assessing knowledge and skills developed through other learning, informal studies or work-related learning, to determine whether the skills and knowledge contribute to meeting the learning outcomes and assessment criteria of a course or program at USC. Completed form can be attached to the application form.
