Health and Safety Incidents

Whenever an employee, student, contractor, visitor or volunteer is involved in, or has observed, a health and safety incident, it must be reported.

A health and safety incident is when an action, object or situation has;

  • caused an injury or illness
  • nearly caused an injury or illness (near miss) or
  • the potential to cause injury or illness (hazard or unsafe behaviour).

Reporting an Incident

All incidents should be reported to a supervisor/manager immediately to ensure appropriate and timely actions are initiated including putting in place controls to help prevent further injury or illness.

If you notice something that has the potential to cause a physical or psychological injury or illness (a safety hazard or unsafe behaviour) this should also be reported through the online report system to help protect yourself or others from harm.

An incident report should be submitted as soon as practicable (ideally with-in 48 hours).
