Only use this form to request a leave of absence if you have had enrolment after Census Date in at least one study period. Refer to thinking of changing or leaving your program for guidelines on leave of absence.
If it is before Census Date in your first semester of study, do not complete this form. Refer to defer your place to see if you are eligible to defer and the process for deferring your place.
